To apply  for these OMRDD services

       You  will need to provide the following documentation ;

A copy of the most recent IEP

A current psychological with an IQ and a Vineland score which documents a substantial handicapping disability.

A current psychosocial with development history documenting a person was diagnosed with a developmental disability prior to age 22 years

A copy of the person Benefits/Medicaid card

Social Security card

A current physical from with a ppd or ppd status

Welcome to BLOCK INSTITUTE, formerly Brooklyn School for Special Children (BSSC), Community Residential Opportunities for Handicapped (CROH), and Training & Rehabilitation in Useful Service (TRIUS).
