Adult Services: How to Secure Services

The Adult Day Programs serve adults over the age of 18 with a developmental disability who live in Brooklyn or Staten Island and who meet the criteria established by New York State Office of Persons With Developmental Disabilities to receive services. To be eligible to receive Day Habilitation services a person must be enrolled in the Home and Community Based Services (HCBS) Waiver Services and have Medicaid Service Coordination.

Individuals who are interested in Vocational Services should contact the Vocational Services Department, Nick Chirco, Director of Vocational Services, at (718) 947-3200 or nchirco@blockinstitute.org. A vocational services referral form, a current psycho-social evaluation and a psychological evaluation substantiating a developmental disability are part of the screening process.

Individuals participate in a face-to-face screening to determine appropriate service plans and program placement. Individuals who are interested in Residential Services must be eligible for Medicaid and the Home and Community Based Services Waiver.
